Patient FAQs: CCC transition to Village Medical

Q: Who is Village Medical?
A: The transition to Village Medical is more than a name change; it’s a new management team committed to supporting us so we can provide more personalized, high-quality care.
Village Medical has established a tradition of excellence, delivering quality primary care with compassion and kindness. Village Medical offers a network of clinics throughout the United States.

Q: What is the timeline to make the transition to Village Medical?
A: Beginning the last week of October, patients will be informed that Comprehensive Coordinated Care is joining Village Medical. This means I will be closing our current office and relocating to a brand-new Village Medical clinic co-located with Walgreens.
December 5, 2022 will be the official transition date where Comprehensive Coordinated Care officially joins Village Medical and as of December 13, 2022 I will begin seeing patients in our new location at 5001 Ross Ave #120, Dallas, TX 75206 on the northwest corner of Ross and Bennett.
The current Comprehensive Coordinated Care location will close, digital properties such as the website and phone lines will be forwarded to Village Medical to support patient access through this transition.

Q: Will I still be able to see my same provider?
A: Yes, I am joining Village Medical, and you should expect to receive the same level of high-quality care along with added support, enhanced technology, and services from Village Medical.

Q: Will you still accept my insurance?
A: Except for Scott & White Health, I will accept the same insurance plans and look forward to expanding insurance plans accepted in our clinics.

Q: How far is the new Village Medical at Walgreens clinic located from Comprehensive Coordinated Care?
A: Conveniently located 1.7 miles away (or about an 8-minute drive).

Q: Will the Hours of Operation change?
A: The new location offers extended access for patients. I am excited to share you will be able to see a Village Medical provider in our new location during the business hours below:
• Monday – Friday: 7 a.m. – 7 p.m.
• Saturday – Sunday: 9 a.m. – 5 p.m.

Q: Is the phone number changing?
A: The current phone number (214) 824-4585 will remain the same for now, though you will start to see me advertise the Village Medical phone number that reaches our shared services team who can help schedule appointments.

Q: How will our patients be notified about this transition?
A: I will be communicating this transition to patients in multiple ways:
• Waiting-room posters and handouts informing patients of the move
• Mailed patient letters
• Voice recording once the phones are forwarded to Village Medical
• Word of mouth with your help!

Q: Why would Comprehensive Coordinated Care want to join Village Medical?
A: I made the decision based on a number of important factors. Village Medical shares my vision for providing patients with the primary care they deserve in a coordinated, accessible way. Village Medical is also focused on primary care and how they can continue to improve care and services while also making healthcare accessible. It also means more time for your physician in the clinic caring for patients.

Q: What new services will be available?
A: Our services will remain the same and may expand. Village Medical offers a wide range of primary care services and focuses on treating chronic conditions such as COPD and heart disease, just like Comprehensive Coordinated Care. Village Medical will also offer at-home services at a future date in 2023.

Q: What happens to my prescriptions?
A: Your prescriptions can be received the same way you always have. Although Village Medical has a partnership with Walgreens, patients will always have a choice of pharmacy.

Q: What happens with my current Medical Record at Comprehensive Coordinated Care?
A: Your medical records will be automatically transferred securely to our new Village Medical location. Give us a call at (214) 824-4585 to obtain a copy of your medical records, or if you wish to have your medical records transferred elsewhere.

Q: What if I have a Billing question for a past service with Comprehensive Coordinated Care?
A: You can continue to call my current number (214) 824-4585 for billing questions related to medical services provided prior to December 5, 2022.

Q: Will my current scheduled appointments be honored?
A: We will remain on our current system for scheduling for some time as we transition to a new portal and system, all appointments will be transferred over and honored.

Q: Will the process for scheduling appointments change?
A: For now, please continue to call Comprehensive Coordinated Care to schedule future appointments. Any previously scheduled appointment occurring after December 5th will transfer automatically to Village Medical’s medical record system where you can expect a Village patient service representative to provide you an appointment reminder call with the time and new location.

Q: Will I have to fill out new patient paperwork?
A: Yes. We will request that you update a few pieces of information as you will be considered a new patient to Village Medical, but our staff will handle that with you at your appointment.

Q: Where will patients be sent for labs, radiology, and other testing needs?
A: For the time being, we will stay on our same portal and records system. This means changes will not be immediate. We look forward to communicating any changes in the coming months.

Q: Will policies or requirements for patients change?
A: Some policies and requirements for patients will change. If this applies to you, you will be contacted directly by your care team.
